Schedulista's booking form asks for your client's name and contact information to simply and quickly help them through the scheduling process.
You can also add custom fields to ask clients for additional information while booking that is specific to your business or organization.
Custom fields can be added to all services or to specific services. There are no limits to the number of custom fields you can add to a service. Though, we recommend adding only a few select fields to not make the booking experience too cumbersome for your clients.

Notes on styling text for prompts
You can use limited HTML markup to style the text in the Prompt field on a custom field. The Prompt is the question the client sees in the online scheduler to provide an answer.
To style your text, the following markup is available:
Bold (<b>)
Strong Importance (<strong>)
Italicize (<i>)
Emphasize (<em>)
Underline (<u>)
Strikethrough (<s>)

Creating a custom field
We offer seven (7) types of custom form fields, six (6) types to support different kinds of responses to questions and the seventh -- a custom notice -- to provide information to clients.
(1) On the web app: Go to Settings > Custom Form Fields
On the mobile app (version 7.3 or above): Tap on Custom Form Fields under the Configure section on the Settings tab
(2) Click or tap Add Custom Field
(3) On the field details page, select your field Type from the drop-down menu
(4) Enter a Field name
Note: This is an internal name that will appear in the appointment details on your calendar. Clients won't see the field name.
(5) In the Prompt field, enter the text of the question to ask the client
(6) (optional) Check the Client response required? box if you want to make the question necessary for the client to answer to schedule their appointment
(7a) In the Services section, select to apply the custom field to All services; or
(7b) Select to apply the custom field to Select services.
You will be able to choose which service(s) to apply the custom field to from the menu.
(8) On the web app: Click Save
On the mobile app: Tap Save at the bottom of the page, then tap Close at the top to return to the Settings tab
Ordering or re-ordering your custom fields
On your Custom Form Fields settings page, you can order or re-order how your custom fields and notices will appear on the details form in the scheduler.
On the web app: Go to Settings > Custom Form Fields
On the mobile app (version 7.3 or above): Tap on Custom Form Fields under the Configure section on the Settings tab
On the web app: Click and drag the field/notice tiles on the three lines on the left to change the order.
On the mobile app: Press and hold on the field/notice tiles on the three lines on the left, then drag to change the order.
Hiding a custom field from the client
You might want a custom field that can appear in the appointment information on your calendar that you don't want the client to see in the scheduler.
You can then use the field internally to include information about the client on the appointment that you don't want to include in the appointment notes.
While adding or editing a custom field, click or tap the green Yes button next to Shown in scheduler? to change it to No. This will hide the Prompt text. Then, click or tap Save.
What clients will see
The custom fields and notices will appear below the client contact fields on the details form in the scheduler. If you take payments or capture credit card details while scheduling, the custom fields and notices will appear below those fields as well.
What you'll see
The custom fields responses will appear below the client contact information in the full appointment details on your calendar.
The Field Name for each custom field will appear with the client's response next to it.
Click or tap the green pencil at the end of each custom field response to make changes, if needed.
Note: Custom field data also appears in appointment reports that are exported to a file.
